INSTRUCTIONS FOR EYE-SCREENING

Please print out the recording form below and complete only the top portion of the form.
Your completed Recording Form is your only QuickPass for the eye screening. As soon as you arrive at the venue, please submit your form at the Registration Counter and take a seat to wait for your name to be called.

NoFor your convenience, recording forms will also be available at the registration counter, but priority will ONLY be given to participants who preregistered and printout their own forms. This is the only quickpass for you to be attended with the least waiting time.


FAQ

Note that by attending the Eye Screening, you are deemed to have AGREED to the following information in full.

IGARD reserves the rights to reject participants who do not comply with the terms and conditions set forth below.

 

1. Is there any age limit for the Eye Screening?

No. There is no age limit for adults, but children age 12 years old and below will not be eligible for the eye screening. The eye screening will particularly benefit teenagers, young adults, middle-aged adults and elderly.

Parents are strongly adviced to bring their children to attend a PAEDIATRIC EYE EXAMINATION. (See IGARD Website www.igard.com.sg)  

2. How long does the Eye Screening take?

It will normally take 20 - 30 minutes to go through all the stations for the many different tests we offer to check your eyes. However, please expect that there may be some waiting time in between each station especially during peak hours. 

Note that your registration time is NOT the time that you will be seeing the consultant. All participants will be seen based on a first-come-first-serve basis and PRIORITY is given to participants who preregistered AND have printed out their own recording forms. It can take more than 30 minutes during peak hours. We encourage you to bring a book (or any reading material) to keep yourself occupied whilst you wait. Alternatively you are free to leave the queue (and rejoin 10-15min later) after completing Station 3.

3. Can I have the Eye Screening wearing my contact lenses?

Yes, you can have your Eye Screening wearing your contact lenses. You may not need to remove your contact lenses if you wish to skip some stations (e.g. glaucoma screening). In fact, you may also wish to get your current contact lenses checked too. So, wear them! Get them checked!

If you do not intend to skip any station, please do not wear your contact lenses at least 4 hours prior to the eye screening. You will only need to skip Glaucoma Screening if you do not remove your contact lenses.

If you are planning to get a new supply of contact lenses, you may wish to PRE-ORDER contact lenses via email/phone to enjoy great savings. Cornea Health Assessment will be offered FREE to participants who pre-order contact lenses. Quote the NAME OF YOUR INSTITUTION/COMPANY. University pricelist is available for download at www.igard.com.sg/ntu or www.igard.com.sg/nus  

4. Will I receive a report?

No. A report will not be provided. However, a short Visual Health Consultation at the end of the screening will give you the chance to discuss your results. All abnormal results found in the screening will be highlighted to you on the spot and you will be advised accordingly.

Note that each participant are only given a very limited time for their Visual Health Consultation. Please leave the station once the consultant has completed your consultation. If you need more time than others, you will strongly be advised to attend a separate appointment for further consultation. In the rare and unlikely case where you do require a copy of your test report, you must only request from our centre. An administrative fee will be imposed for each copy of your test report and it must be collected personally. Our staff will not be able to scribble any test results on paper for you during the eye screening. Please also note that your recording form must be retained by our staff at the end of the Eye Screening. 

5. I've got perfect eyesight. Do I really need an Eye Screening?

Yes. You are advised to come for an Eye Screening on a regular basis. Perfect eyesight does not guarantee you perfect visual health. Eye problems can creep in without any early warning symptoms such as a deterioration of eyesight. Prevention is better than cure.

6. My doctor said that I've got a very serious cataract. Can I get my eyes checked again?

Yes. It's always good to get a second opinion, but it does not mean that your cataract will get any better by checking it again and again. Please take your doctor's advice seriously and get your eyes treated. 

Please inform the consultant about your eye condition at your earliest opportunity. You can bring your old prescription or eye examination report along so that our consultant can have a quick look at it. Note that the Free Eye Screening is meant to pick up early problems and not meant for treating diseases.  

7. Do I need to prepare myself for the Eye Screening? Will my eyes be dilated?

No, you will not need to prepare for the Eye Screening. Just come in!

Your eyes will not be dilated in this Eye Screening.

8. I've forgotten when I've made my online booking. Can I check it?

You will receive an email once you register online. However, you can email tina@igard.com.sg if you fail to receive this email. Please provide your Full Name, NRIC/Passport Number and Mobile Phone Number so that we can verify your information. 

9. I need to reschedule/cancel my booking. How can I do that?

To reschedule a booking, please re-register online (on the relevant webpage) with your new Preferred Date/Time. Please indicate "ignore previous booking" in the Remarks field.

To cancel a booking, please email tina@IGARD.com.sg with "Cancel Booking" in the subject line. For security reason, you will also need to supply your Full Name and NRIC/Passport Number

Note that we will not release information to anyone without your consent.    

10. Can I bring my family &/or friends along without making an online booking?

No, the Free Eye Screening is strictly for participants who have booked online only. However, we will offer your family members (or friends) a Free Eye Screening (Basic) if you PRE-REGISTER them for Retinal Health Screening at a very nominal fee of $8. Have fun!

Usual price of Retinal Health Screening is $35 for university students/staff. Each participant is limited to register a maximum of 4 family members/friends provided that they have also pre-registered for themselves. Note that pre-registration for Retinal Health Screening is only entitled to participants who have attended the Free Eye Screening. Available during the event only.  

11. Do I need to bring my spectacles or contact lenses?

Yes. Please bring your spectacles (or your current prescription) along. If you wear contact lenses, please bring your old contact lens prescription (or empty box) along. This information will enable our consultant to advice you if your old prescription need to be updated.

If you like to buy spectacles or contact lenses from IGARD, our consultant will be able to conduct an additional test known as Ocular Refraction for you. Note that the Free Eye Screening does not entitle you to any additional test or prescription. Additional charges will apply if you request for additional services that are not covered under the Free Eye Screening.   

12. I have problem using your website to register. What shall I do?

If you have problem using our website to register, you may like to email tina@igard.com.sg with "Manual Eye Screening Registration" under subject heading. Include your Full Name, Mobile Phone Number, School/Department, Preferred Date, Preferred Time, Date of Birth and NRIC/Passport Number. Note that this mode of registration is not encouraged because priority will only be given to participants who register directly via website.

13. I have missed the Free Eye Screening. What shall I do?

Please attend a Standard Eye Examination. You can book for an IGARD appointment at Orchard, NTU or NUS at reduced prices. Please book appointment via www.igard.com.sg

14. My eyes are red today. Can I attend the Free Eye Screening?

The aim of the eye screening is to pick up early signs of abnormalities, eye diseases and small early changes in eyesight. Early detection is important in Preventive Visual Health. Free Eye Screening is not meant for diagnosing or treating eye abnormalities. If your eyes are red, please seek for professional advice and treatment at a clinic.

15. I have never tried contact lenses before, and I really want to try. What shall I do?

Please let us know as soon as you arrive. There is a Contact Lens Starters Programme which is available for first-time contact lens users only. You can register for this at a VERY nominal price of $12 only during the Free Eye Screening event.
